Safety management incident/accident/Hazard reporting system

Using the class-developed reporting from, all students will submit to the project management committee online at least two hazard, incident, accident reports prior to November 15.

Physical safety is a primary objective of any SMS. However, reports will include all matters related to the success of individuals and to the welfare of our class. Therefore an incomplete, late, or failed assignment or exam should be self-reported.

Non-punitive reporting is essential, so self-reporting as soon as you identify a problem is to your advantage. By being pro-active in solving your problems you are showing a professional attitude. Your colleagues and instructors will be part of a Corrective Action Plan (CAP) that will work to help you solve academic or deportment problems.

Instructors are also invited to file reports.
